Insured shall be a Filipino citizen and the registered applicant of Ka Barako Insurance. Valid for ages 18 to 60 years old (renewable up to 65 years old). Must be in good health and actively at work.
Valid for one(1) year from the date of activation via KA BARAKO website.
Beneficiary must be the LEGAL heirs of the insured.
- For married insured, the beneficiary shall be the legal spouse or the children.
- For insured who are single, beneficiaries shall be the parents or siblings.
LEGAL names of the insured and her beneficiaries, as well as other data submitted MUST be accurate for the insurance to be valid.
For accidental/natural Death, the beneficiaries shall receive the maximum coverage of this policy.
For injuries or permanent disablement caused by accidents, the insured shall receive up to 100% coverage depending on the extent of the injury. Please refer to the Table of Benefits for details.
The Medical Reimbursement benefit covers medical expenses with supporting receipts related to accidents covered under Ka Barako Personal Accident Card. It also covers medical expenses incurred from animal bites. This benefit can only be claimed once during the year of coverage.
The Insured is covered for disability or loss of life due to unprovoked murder and/or assault arising from robbery, hold-up assault, and other unlawful offenses done. Unprovoked means that the insured should not have done any action or issued words to encourage any harm done to themselves by the assailant.
24-hour personal accident insurance coverage for one (1) year, anywhere in the Republic of the Philippines and the civilized world (subject to Department of Foreign Affairs Travel Advisories)
While in the course of activities, whether within or outside work premises (except stated in general exclusions).
While traveling in public or private conveyance used for land and sea travel and while traveling solely as a passenger on any commercial airline flights duly licensed to carry passengers for hire.
NEL: No Evidence Limit is P25,000.00 provided the eligible individual has not attained his 65th birth anniversary.
Accidental/Natural Death or hospitalization that happened in other countries are covered through a Reimbursement basis.
50% Coverage for motorcycling accidents.
This insurance shall be considered null and void if the accident or the unprovoked murder and/or assault occurred while the Insured is participating in any criminal act or related offence.

“LOSS” as used above within reference to hand or foot or thumb means complete severance through or above the wrist or ankle joint or both phalanges respectively; as used with reference to eyes, means the entire and irrecoverable loss of sight.

Where the injury is not specified, the Company will adopt a percentage of disablement, which in its opinion is not inconsistent with the provisions of the Schedule of Indemnity. In the event of multiple injuries compensable under this Section, the liability of the Company shall only be the injury requiring the highest indemnity.

GENERAL EXCLUSIONS:
This policy shall not extend to cover:

Death or disablement occasioned by and happening through

War, invasion, act of foreign enemy, hostilities (whether war be declared or not), Civil War, Rebellion, Insurrection, Mutiny, Military or Usurped Power, violence occurring in any Assembly or Demonstration, Civil Commotion, Riots, Strikes, Military or Popular Rising and Terrorism.
Suicide, Attempted Suicide (whether felonious or not, sane or insane), alcoholism or insanity
Suicide, Attempted Suicide (whether felonious or not, sane or insane), alcoholism or insanity
Poison or any poisonous substance accidentally or otherwise taken, administered, absorbed or inhaled during addiction and the like.

Death or disablement or bodily injury, occurring whilst the insured is traveling in an aircraft other than the one licensed for public passenger service and operated by a regular Airline on a published scheduled flight over a regular air route between two definitely established airports in which the insured is traveling as a ticket-holding passenger.
Death or disablement or bodily injury consequent upon the insured engaging in hunting, racing of all kinds, steeple chasing, polo playing, motor racing, mountaineering, winter sports, ice hockey, football, and yachting, underground mining, all military and police personnel and officers, barangay officials/Tanod, Natural Gas and Oil Industry, Professional entertainers & athletes, Demolition or Explosives, Underground workers, Firemen, Scaffolders, Political Groups, Pilots, Metro Aide
People who have the following pre-existing medical conditions: Total Blindness, Total Deafness, Skull or Spinal Injuries, Infirmities, and Chronic Disease
Unprovoked Murder or Assault that occurs in any of the following provinces or places:
Death, disablement or bodily injury whilst attempting to do or doing an illegal activities or operation.

Compensation shall be payable only when the entire amount of the claim shall have been ascertained and proved to the satisfaction of the Company.

NOTICE OF CLAIM
Written notice or any advice on which claim may be based must be given to the Company within thirty (30 days) after the date of the accident causing such injury. In the event of accidental death, immediate notice thereof must be given to the Company. Hereunder are the claims requirements, to wit:

Copy of the Insurance Certificate
Medical Certificate (original copy)
Medical Receipts
Hospital Bill
